Update on AAIB investigation into Leonardo AW189 helicopter incident

The AAIB investigation into a major incident involving a Leonardo AW189 helicopter, GMCGT, at Heads of Ayr, South Ayrshire, on July 30, 2021, is updated in this announcement.

The Automatic Flight Control System’s Search and Rescue (SAR) modes caused pitch instability and an incorrect status indication for the flight crew during an offshore training sortie (AFCS).

The AAIB study has concentrated on how the AFCS SAR modes function and the cues it gives flight crews.

To inform flight crews of this problem, the helicopter manufacturer has released a Technical Information Letter.

The report is almost finished, and then it will be released.
